What are 1st & 2nd generation of Laser Eye Surgery?

The first generation of laser eye surgery was initially called PRK or LASEK.The laser is applied directly to the surface of the eye.This results in quite an amount of discomfort and watering and a slow visual recovery.For that reason,a second generation of laser eye surgery called LASIK was developed.The first step is to create a flap on the surface of cornea which is peeled open.The second step is to use a laser to sculpt the glasses prescription into the cornea,thereby correcting the vision.The flap is then placed back & vision recovers rapidly within 24 hrs with minimal discomfort & only mild watering from the eye.

How does lens replacement work?

In lens replacement (RLE) we are removing the natural lens before it ages into cataract.After removing lens,eye is out of focus & lens implant is used to focus the eye.Lens implants come in all sort of different shape,size,strengths & different focusing abilities.Usually when we perform refractive lens exchange,we use multifocal lens implant to correct the eye & we get all ranges of visions i.e near,intermediate& distance with multifocal lens.

The reason why people choose to have laser surgery is that they find that consequences of wearing glasses or contact lens is very disabling or they have a huge negative impact on the quality of their lives.People who do sports find it impossible to wear glasses.People who wear contact lens cannot swim because it is not safe to do that.Patient who wear contact lens for years may become intolerant to their contact lenses i.e eye becomes inflammed or uncomfortable or they may have gritty sore eyes at the end of the day.This is very common reason for people seeking laser eye surgery.

How does the eye work?

Eye works like a camera.The part of eye which senses the light is retina & there is a thin layer of nerve cells at the very back of eye.But in order to see clearly, we have to have an in focus image & that is where the cornea and the lens of eye come in.The cornea of eye is front window of eye & this does 2/3rd of focusing of light onto the retina.The final 1/3rd of focusing and also the zooming mechanism of eye are performed by the lens which is inside the eye.The lens is capable of moving & focusing at different distances & fine tuning the sharpness of image onto the surface of the retina.

The lens is one of the most important structures in eye.It is responsible for focusing and fine tuning the sharpness of the image on the retina.Lens enables us to focus eye at various distances i.e it enables us to look at far distance one minute & next minute look at very near distance.The ability to change the focal point of eye is critically dependent upon the healthy lens.

What is cornea?

The Cornea is the front clear window at the very front part of eye.It is highly specialised structure that gives strength & protection to the eye and prevents any bacteria from entering the eye.The other important role it fulfils is to focus the light,it provides two thirds of the focusing power of the eye.The clarity of the cornea is critical for vision.If cornea turns cloudy,you are looking through misty window.

Why do we need glasses or contact lenses?

People who are short sighted or myopic have much focusing power,so need to wear a negative power lens in front of the eye in order to bring image into focus on the retina.People who are long sighted do not have enough focusing,so in order to bring the image into focus on the retina,they need to wear a plus lens in front eye.Lastly,astigmatism is a condition where eye rather being completely circular has an oval shape to it and requires an oval shape or what is called a toric lens to be worn in front of the eye to bring the world into focus.

What are alternatives for those not eligible for Laser Eye surgery?

The alternatives to laser eye surgery are implantable contact lenses(ICL) which are placed behind the iris on the surface of natural lens and correct the glass prescription.The second option is that if patient is Presbyopic i.e is not able to read clearly & is not suitable for laser surgery,the option for this patient is refractive lens exchange (RLE) where natural lens is removed & replaced with multifocal lens implant to get good distance, intermediate & close up vision.

How do you assess a patient’s eligibility for Laser Eye Surgery?

We assess the suitability of someone for laser eye surgery in 3 steps:- 1) First step is to have sophisticated scans which examine the health of the eye but in particular focus of the cornea to see whether cornea is sufficiently strong to withstand effects of laser eye surgery. 2) The second step is to see one of our optometrists who is specialist in refraction & prescribing glasses.He gives you correct prescription & sees that there has been no chance that could affect outcome of laser eye surgery. 3) The third step is for you to see a consultant ophthalmologist who ultimately is responsible for your care & for performing surgery on you .He will discuss your scans,check your prescription ,ensure that it is safe for you to have laser eye surgery & help you decide which form of laser eye surgery is correct for you.In some cases laser eye surgery is not suitable for you & we have other alternatives as ICL which the consultant ophthalmologist will discuss with you.

What happens on the day of laser surgery?

On the day of laser surgery ,you come to the hospital an hour before surgery, you will be seen by nurses and they will check your B.P & heart rate.If you are anxious ,you are given a mild sedative in the form of tablet.It is very mild but very effective at calming the patients & making them feel comfortable & less anxious. About 20 mins before that start of surgery you are taken up where our laser systems are located .We will lie you flat on bed and we will swing you underneath the laser.We then start applying some anaesthetic drops.These are very effective eye drops.They rapidly work to numb whole surface of eye & edges of the eyelid.We use very gentle speculum to hold eyes open.With speculum your eyelid do not meet & device keeps them out of the field we are working in.Now you look at the flashing light and we complete the procedure whether LASIK or LASEK.The whole procedure takes less than five minutes per eye & laser works for only 20-30 seconds per eye depending on your prescription.After completing the procedure we put in drops to help the eye heal .We do one eye first & then we proceed to the treatment of the second eye & once both eyes are done we bring you back,give you a cup of tea & give you a final check before you go home.
